Cytokines and Chemokines at the Crossroads of Neuroinflammation, Neurodegeneration, and Neuropathic Pain

Abstract

Cytokines and chemokines are proteins that coordinate the immune response throughout the body. The dysregulation of cytokines and chemokines is a central feature in the development of neuroinflammation, neurodegeneration, and demyelination both in the central and peripheral nervous systems and in conditions of neuropathic pain. Pathological states within the nervous system can lead to activation of microglia. The latter may mediate neuronal and glial cell injury and death through production of proinflammatory factors such as cytokines and chemokines. These then help to mobilize the adaptive immune response. Although inflammation may induce beneficial effects such as pathogen clearance and phagocytosis of apoptotic cells, uncontrolled inflammation can result in detrimental outcomes via the production of neurotoxic factors that exacerbate neurodegenerative pathology. In states of prolonged inflammation, continual activation and recruitment of effector cells can establish a feedback loop that perpetuates inflammation and ultimately results in neuronal injury. A critical balance between repair and proinflammatory factors determines the outcome of a neurodegenerative process. This review will focus on how cytokines and chemokines affect neuroinflammation and disease pathogenesis in bacterial meningitis and brain abscesses, Lyme neuroborreliosis, human immunodeficiency virus encephalitis, and neuropathic pain.

Dat is idd een interessant artikel, jammer dat mijn kennis van biologie te kort schiet...
Maar dit lijkt wel wat van de kern van ons ziekteproces te raken?!?

Ik ben ook maar een amateurtje, maar ik ben het helemaal met je eens

Wat ik het interessante vind van dit artikel is dat er van twee kanten gekeken wordt naar cytokines. Inflammatie wordt vaak alleen belicht als schadelijk, maar heeft natuurlijk ook een functie. Hier wordt ook aandacht besteed aan het nut ervan. En veel cytokines die pro-inflammatoir werken blijken ook positieve eigenschappen te hebben zoals het beschermen van neuronen. Het gaat blijkbaar om de juiste balans, een teveel is schadelijk maar ook een tekort.

Het idee voor de toekomst:
"Further, it will be a challenge to design therapeutic agents that safely and effectively target only the detrimental mechanisms that contribute to disease pathogenesis, as cytokines and chemokines are vital for the normal functioning of the body."

Wat betreft neuroborreliose wordt er naar mijn idee verder niet ingegaan op de manier waarop Borrelia zelf het immuunsysteem onderdrukt. Ik had graag meer willen weten over hoe de bacterie de afweer manipuleert om te overleven en wat voor invloed dit heeft op de cytokine productie.
Er wordt wel kort aangetipt dat het lichaam zelf op een gegeven moment de afweerreactie kan afremmen om teveel schade te voorkomen. Een soort overlevingsmechanisme, heel interessant. Helaas wordt dit ook niet verder uitgediept, maar het is toch een uitzonderlijk mooi artikel, met inderdaad een heel compleet beeld van neuro-immuunziektes.
Het lukt me niet om de afbeelding te posten, maar het wordt goed uitgelegd in de figuur die erbij hoort.
